Baby Massage and Yoga, and Children's and Family yoga
Full payment is required before the start of your course/1:1 session/workshop in order to secure your date(s)/place. This will be required at the moment of signing in the course/session/workshop (full amount or a deposit). In the event of Wellbeing by Montse cancelling a session/workshop due to sickness, we will rebook the session at a time and date suitable for both parties. In the even of the client .
Sessions as part of a group course, class or workshop cannot be refunded although Wellbeing by Montse is happy to share what you've missed (over a telephone consultation or any way we both agree).
Pre-recorded courses are not refundable, but please do contact us for any issue.
If there is any special cancellation policy, I'll let you know when booking.
In the event of not being a minimum of 2 in a group course, we can either rebook the course or do it as a 1:1 (duration and price might vary).
For 1:1 sessions, if you are able to give at least 48h notice (unless exceptional circumstances) Wellbeing by Montse will endeavour to reschedule this session at a time and date to suit both parties.
Full disclosure of all known and existing medical conditions and/or contraindications are required at time of booking. Any new conditions or contraindications that arise after completing this form should be highlighted to Wellbeing by Montse before your session.
For Baby Massage and Yoga when possible, ensure that any baby immunisations are completed at least 72h prior your booked baby massage session/workshop, as this is contraindicated for massage/yoga until after this time.
Always complete a skin patch test on your baby's wrist when using baby massage oil and follow instructions on the bottle for storage and use.
Photographs will only be taken of you and your baby if you have agreed in the booking form. Any photographs taken in the classes will be used for advertising and marketing purposes only. I may also ask you to take pictures or record a class for learning purposes or to send the pictures/video to you at the end of the course. Parents are permitted to take photographs of their own child but not to others.
Your information details will not be shared with any third parties.
Please allow 10 minutes prior to your treatment to carry out a consultation which is an essential part of your treatment, to enable the therapist to plan a carry out safe and appropriate treatment. Due to Covid-19, I am now doing the consultation forms in advance by mail/zoom. Clients are therefore advised to disclose all known medical conditions, undiagnosed symptoms, medical treatment and medication received when completing this form. Clients presenting with certain health conditions will be asked either to inform their GP or to consult their GP concerning the advisability of the treatment. A GP permission letter will be required. Clients who refuse to inform or consult with their GP prior to treatment do so at their own risk. The therapist reserves the right to withhold treatment from clients presenting with certain health conditions, until the condition has resolved.
This applies in circumstances when giving treatment may pose a risk to the client and/or therapist.
Therapist ensure to treat all information regarding the client as confidential and to only use this information for the purpose for which it was given.
If the therapist considers that disclosure of client information to a third part is appropriate for any reason, he/she will firstly obtain the client's verbal consent.
If you need to cancel your treatment for whatever reason then simply text me to confirm cancellation (07847 588 541). Please note cancellations must be made 24h prior your treatment date, not to be charged.
Payments are accepted by cash or card on the day of the treatment or bank transfer in advance (please, ask me for the bank details).
